When is Hospice Care Appropriate?

Many people mistakenly believe that hospice care is only available when a patient is totally bed-bound and there is a medical emergency.   This is not the case and informed patients often take advantage of hospice services well before there is a medical crisis.  

 

How Do I Determine Eligibility?

We realize that making the transition to end-of-life care can be challenging for both the patient and caregiver.  You don't need to have all of the answers—just  call Transitions—and our specially trained registered nurses will determine eligibility and make the necessary arrangements.

We provide our services on a nondiscriminatory basis and accept patients without regard to gender, national origin, race, creed, sexual orientation, disability, age , place of residence or ability to pay in compliance with the provisions of Title VI of the Civil Rights Act, Title VII of the Age Discrimination Act of 1995, Executive Order 11246 and all other applicable laws.
