At Transitions Hospice each patient and family is assigned a Patient Care Team dedicated to providing responsive, compassionate and professional care. The leader of each team is a Registered Nurse who coordinates a plan of care with our Medical Director, the patient’s attending physician and other team members, to assure our patient’s symptoms are controlled, pain is managed and the patient and family are properly informed.
In addition to a Registered Nurse and a Physician, a Patient Care Team can include: ¨ Certified Nursing Aide who is specially trained to work with the terminally-ill and will provide direct patient care such as dressing and personal hygiene; ¨ Licensed Social Worker who helps with a wide variety of social needs of patients and families ranging from financial considerations to dealing with grief and the loss of a loved one or accessing various community agencies for additional support programs; ¨ Chaplain who addresses the spiritual concerns of patients and family members within each patient’s belief system. ¨ Trained Volunteers who provide companionship and non-medical services for the patient, respite time for the family and support during bereavement; ¨ On-Call Nursing Team who are always available after hours and on weekends for visits or phone consultation; and ¨ Other Specialists, such as nutritionists and physical, speech or occupational therapists, may be added to a patient’s team as needed. |
