How it Works

Take the first step

Call us and schedule a consultation with our team, either by self-referral or through a physician’s referral or recommendation.

Our discussion will focus on your loved one as we figure out how we can best serve you. 

Step 2

Getting admitted

We’ll conduct an in-home medical evaluation, obtain medical records from your doctor, then progress into admission.

We’ll be by your side, helping you figure out what questions to ask your doctor, and helping you best prepare for the next step. 

Step 3

Receive quality care

Our thorough process allows us to meet any need you might have. We admit your loved one, place medical equipment and prescriptions, establish support schedules, administer hospice care, and supply any other financial or community resources you’d find helpful.

After that, we’re here to provide help, love, and care as long as your loved one needs that level of support. 

Our Services

Hospice care is more than healthcare

We’re here to help with anything you might need, even outside of care, medication, and equipment. 

Companionship

Transitions Hospice will provide volunteer services to those patients and families that request one.

Domestic Support

A Hospice Certified Nursing Assistant (C.N.A.)  provides basic personal care for the hospice patient.

Equipment & Supplies

Transitions Hospice provides all medical equipment related to the hospice diagnosis. This may include a hospital bed, wheelchair, oxygen, etc.
